HIPAA notice

How your health information is protected, and where to find the official Notice of Privacy Practices.

Protected health information

Health information created during your care is protected by the Health Insurance Portability and Accountability Act (HIPAA). You have rights to see and get a copy of your records, ask for corrections, and learn how your information is used and shared.

Official Notice of Privacy Practices

Your care is provided through Capital Diabetes & Endocrine Associates. The practice’s Notice of Privacy Practices explains how your medical information may be used and disclosed. You can request a copy from the practice at either office; its forms (including a HIPAA form) are linked in the practice details. This summary does not replace the practice’s official notice.

This website

This website is informational. It does not store medical records. Please do not send health information through the website form; use the practice’s secure patient portal instead.
