Visit guide

Plan your visit

A clear path from first contact to follow-up—what to check, what to bring and how the patient portal fits in.

  1. 1

    Check your coverage

Call your insurer’s member services to confirm the practice is in network. HMO plans and several others need a referral from your primary care provider. The practice’s insurance list is linked in the practice details on the about page.

  2. 2

    Get in touch

Send a note through the contact page or email [email protected] to start scheduling. Established patients can also book through the practice’s patient portal (linked in the practice details).

  3. 3

    Complete new-patient forms

The practice offers a New Patient Registration Form and Medical History Form—each in English and Spanish—plus its office policy and records-release forms. You’ll find the link in the practice details.

  4. 4

    Gather what to bring

Use the checklist below. Having your medicines and glucose data on hand makes the first visit much more useful.

  5. 5

    Visit day

For office visits, arrive a little early. Televisits use the practice’s secure video system; joining instructions are linked in the practice details.

  6. 6

    After your visit

Use the practice’s secure patient portal for messages, results and your next booking. Complex prior authorizations (mostly for weight-loss medicines) need a scheduled appointment, per the practice’s policy since September 1, 2025.
